Hello Wales Families,
The Mobile Dentist will be coming to Wales Elementary School on December 4th, 2025. The program is not cost to you, and is covered by your dental plan. You have two options for signing your student up. You can complete the paperwork that was sent home with your student. Option 2, you can use the link provided below to sign your student up. Please sign your student up online or return the paper form as soon as possible.

Best of all, in most cases there is no cost to you if your child is insured with Medicaid or CHIP! Private insurance and affordable self-pay options are also available.

Having your child seen by the in-school dentist saves you valuable time. Top quality care is provided by local, licensed dentists right in the familiar surroundings of your child’s school. Dental care includes a complete exam, x-rays, cleaning, fluoride and sealants when needed. Restorative care, such as fillings, may also be provided.
