Welcome to the Nurse's Corner!

Below are reminders:

-This is a nut-free school. Please do not send nut products to school with your child.

-It’s a requirement to have new physical forms submitted for every student entering pre-school, kindergarten, 4th grade, and any student transferring from another district. So if you have not already done so, please submit these documents as soon as possible. All students must have a copy of their immunization record and birth certificate on file as well as the physical form.

-If your child takes a prescription medication at school, the Parent Permission Form and Medication Order Form must be filled out before I can administer medication to your child. 

-Please return your Over the Counter consent forms to Nurse Natalie ASAP!
